Making a Complaint & Giving Feedback
Pro Perform Physiotherapy welcomes feedback from all of our patients.
Patients may give feedback in person, via email or over the phone (08) 9309 3166. If patients wish to remain anonymous they can fill out our online form and leave their details blank.

Reporting an Incident
As an NDIS provider, Pro Perform Physiotherapy is required to have an incident management plan in place. The following document details what constitutes a reportable incident and the process to take if an incident does occur.
