EPC Referrals

What is an EPC Plan?

An Enhanced Primary Care (EPC) plan -  also called a Chronic Disease Management (CDM) plan -  is a GP referral program that provides access to up to 5 Medicare-rebated physiotherapy sessions per calendar year.

EPC physiotherapy is available to people with chronic conditions or musculoskeletal pain that would benefit from ongoing treatment.

How Does It Work?

  1. Visit Your GP

    • Speak with your GP about your condition and eligibility.

    • If suitable, your GP will create a care plan allocating physiotherapy sessions.

  2. Referral to Pro Perform Physio

    • Your GP can fax the referral directly to us, or you can bring the paperwork into the clinic.

  3. Book Your Appointment

    • Once the referral is received, our admin team will contact you to arrange your initial physio session.

Gap Payment Information

At Pro Perform Physiotherapy in Woodvale, a $20 gap fee applies per EPC visit. This allows us to provide high-quality, evidence-based care while still making physiotherapy accessible through the Medicare rebate.
